I got the information I needed from another forum anyway.  The best place for an American to cash out in my mind is either PICClub or eWalletXpress.  PICClub has a fee on money going in, but is free on money withdrawn.  The PICClub fee is a few percent on money going in.  PICClub also offers member tournaments on a very regular basis.  Here's some more information from their site.
